Issuance of Maternity Log

Maternity Log, Pregnancy Health Assistance Certificate and General Health Checkup Form for Infants will be issued.

Consultation about pregnancy, childbirth and child-care will be provided as well.

Every week on Mondays (except for national holidays)

If you apply yourself

1. Personal registered (unregistered) seal of the pregnant person
2. Individual Number / Verifying Documents
   Individual Number Card, Notification Card, copy of Residence Certificate mentioning your Individual Number
3. Verifying documents of the pregnant person.
   Official documents with a photo attached (Individual Number Card, driver`s license, passport)
   In case you should have difficulties bringing the above, please bring your health insurance card and pension book.

If someone else applies in your stead

1. Personal registered (unregistered) seal of the pregnant person
2. Individual Number / Verifying Documents
   Individual Number Card, Notification Card, copy of Residence Certificate mentioning your Individual Number
3. Verifying documents of the applicant (proxy)
   Official documents with photo attached (Individual Number Card, driver`s license, passport)
   In case you should have difficulties bringing the above, please bring your health insurance card and pension book.
4. Authorization (the pregnant person authorizing the proxy to act in her stead)
5. Personal registered (unregistered) seal of the proxy

Public health nurse

1. Issuance will be conducted at place of residence
2. A schedule is provided in case public health nurses are unable to carry out physical examinations etc. If you cannot attend during the consultation hours, please contact us in advance.
3. Please fill in gestation age and due date into the Pregnancy Notification Form
4. From January 1, 2016 onwards the confirmation of your Individual Number is made obligatory for issuing the Maternity Log.
